Cause : Supply Phase comes when the drive has detected a supply phase loss or large supply imbalance. Supply phase imbalance occurs when there's an unequal distribution of voltage or current among the phases of a three-phase electrical power system. 

1. Check the A.C. supply voltage balance and level at full load.
2. Check the output current stability.
3. Reduce the duty cycle.
4. Reduce the motor load.
